To All Prospective Student-Athletes and their Parents/Guardians
Student Athletes are required to submit the following forms PRIOR TO competing as student-athletes at Eastern. Download the Medical Form and Insurance Letter and follow the instructions closely. Fall Sport Student-Athletes need to have the forms complete by August 1. The deadline for Winter and Spring Sport Student-Athletes is August 15th.

ADHD Documentation Form
Athletes diagnosed with Attention Deficit Hyperactivity Disorder (ADHD):
If you have been diagnosed with and are being treated for ADHD, new NCAA regulations require you to do the following: obtain documentation that demonstrates you have undergone a clinical assessment to diagnose ADHD, are being monitored routinely for any use of medication to treat the disorder, and have a current prescription on file.
These steps must be complete in order to receive a medical exception to the NCAA Banned Drug Policy. Download the form below to have your physician review and complete. Bring this completed form with you to pre-season camp or mail to the athletic department (Attention: John Post). Sickle Cell Trait Information Release Form
This is a required form for those student-athletes with this condition. Please indicate this condition on your medical history paperwork. Also, have your diagnosing physician send the results and confirmation of this condition to the Eastern University Sports Medicine staff.

Service Learning Letter
All- first year students and transfers have a service learning requirement for INST 150. Athletes have the opportunity to complete the 20 hour requirement prior to the start of the school year. There are very specific instructions on service learning in the attached letter. It is highly recommended that all athletes take advantage of this opportunity to complete hours early. Student Athlete Travel Release Form
Any time a Student-Athlete desires to travel to or from an Athletic Event in a non-team vehicle, the coach must give consent and have following form complete.
